Prior to volunteering, every volunteer must go through a background check. Please return the following form to the campus office where you will be volunteering:
1. Completed Background Check Permission form. Please complete ONLY Sections C and D.
2. Please DO NOT attach any payment.
3. Please DO NOT mail the form to the WSP…return it to the Office Manager at your Campus.
4. If you wish a print-out of the results, include a stamped, self-addressed envelope with the completed WSP form.
Background checks are required for updating every two years. One background form completed will cover volunteering at all campuses.
