Welcome to the 09-10 online back to school forms & information
Below is a check list to help you to be sure you bring all the required forms to the Mack Office on or before the first day of school!

FOR ALL FAMILIES
MUST READ:
FORMS REQUIRED FOR THE FIRST DAY OF SCHOOL
- Authorized Driver / Carpool form
- Milk order form with check attached
- After School Program Registration & Contract
- Jog-A-Thon information & student/parent t-shirt order form (Due September 3rd!)
- Background Check Form for all volunteering parents (done every 2 years, please include a self-addressed stamped envelope)
- Scrip registration form and order if desired
FOR ALL STUDENTS NEEDING ANY MEDICATION, PRESCRIPTION OR OVER THE COUNTER WHILE AT SCHOOL
- Oral Medications Form and medications —if your student will be taking ANY medication during the school day, this form must be completed by you and your physician and the medications brought with the form to the Mack Office.
