Below is a check list to help you to be sure you have all the information you need and know all the required forms that are to be brought in to the Mack Elementary Office on or before the first day of school!

FOR ALL FAMILIES

MUST READ:

• Peanut Allergy Policy

FORMS REQUIRED FOR THE FIRST DAY OF SCHOOL

• Authorized Driver / Carpool form
• Milk order form with check attached
• Background Check Form for all volunteering parents
• Scrip registration form and order if desired

FOR ALL STUDENTS NEEDING ANY MEDICATION, PRESCRIPTION OR OVER THE COUNTER WHILE AT SCHOOL

• Oral Medications Form and medications —if your student will be taking ANY medication during the school day, this form must be completed by you and your physician and the medications brought with the form to the Mack Office.

FOR ALL 6TH GRADE FAMILIES

• 6th Grade Varicella/Chicken Pox AND latest tetanus date form
