To apply to be a team member of a 2012 Service Outreach Trip, just open the PDF application of your choice (see links below), print and complete, and submit by December 15th to the HS office. This has been changed from the original December 9th deadline. For applications submitted after the December 15th deadline, a fee of $50 will be applied.
